The CSB noted that multiple significant process safety deficiencies were recognized during this incident, particularly in the decommissioning phase of operations.<\/p>\n<p>The event took place when personnel at the site were disposing of leftover chemicals using the site&#8217;s wastewater pretreatment tank. The procedure initiated with the introduction of two proprietary chemicals\u2014A-50 and M-2000A\u2014both of which were commonly used together in the facility&#8217;s wastewater pretreatment process. A-50, which contains calcium chloride, acted as a coagulant, whereas M-2000A, made up of sodium trithiocarbonate (Na2CS3), was utilized to precipitate silver from wastewater.<\/p>\n<p>Issues emerged when leftover nitric acid was discarded into the tank, unintentionally interacting with the sodium trithiocarbonate to form a hazardous fog filled with hydrogen sulfide. The consequences of this adverse reaction severely impacted the workers involved. Although efforts were made to warn others about the perilous situation, two employees who responded to the event were eventually overwhelmed by the gas and passed away.<\/p>\n<p>A crucial insight from the CSB\u2019s interim evaluation was the detection of process safety lapses at Catalyst Refiners. The Material Safety Data Sheet (MSDS) for M-2000A explicitly indicates its incompatibility with strong acids, including nitric acid. Nevertheless, the facility did not have documented procedures for the safe disposal of this specific combination of chemicals, a deficiency in safety protocols that contributed to the tragic incident.<\/p>\n<p>This occurrence highlights the necessity of strict compliance with safety standards, the importance of precise compatibility evaluations prior to chemical disposal, and the execution of thorough emergency response plans to avert such dangerous events.
